What is a Personal Health Record?
A personal health record (PHR) is a collection of important information that you maintain about your health or the health of someone you’re caring for, such as a parent or a child, which you actively maintain and update.
The American Health Information Management Association demonstrated its advocacy for the empowerment of individuals to manage their healthcare by issuing a joint Position Statement for Consumers of Health Care on the Value of Personal Health Records with the American Medical Informatics Association in February 2007. This form will provide to you a complete, short version of what you will need. For more information about personal health records, also visit AHIMA's website www.myPHR.com.
Check the Calendar of Events page for scheduled "myPHR" presentations in your area.
If you would like to schedule a presentation, please contact a 'myPHR' liaison or marykstrunk@yahoo.com about scheduling a presentation.
