What is a Personal Health Record?

Click on the Icon above to go to myPHR web-site.
A personal health record (PHR) is a collection of important information that you maintain about your health or the health of someone you’re caring for, such as a parent or a child, which you actively maintain and update.
The American Health Information Management Association demonstrated its advocacy for the empowerment of individuals to manage their healthcare by issuing a joint Position Statement for Consumers of Health Care on the Value of Personal Health Records with the American Medical Informatics Association in February 2007. This form will provide to you a complete, short version of what you will need.
Check the Meetings and Events page for scheduled "myPHR or Educational" presentations in your area.
If you would like to schedule a presentation, please fill out our "Contact Us Form" and someone will get back with you right away about scheduling a presentation. Click on this link to go to our Contact Us form - also on the form please let us know a little bit of what you are interested in to help us have the correct person get back with you.
For more information about personal health records, also visit AHIMA's website www.myPHR.com. This site has wonderful information about creating your personal health record and all the benefits it has.
